ACCIDENT REPORTS
These Accident Reports represent a small sample of some of the serious accidents happening around the province. They include non-specific details from WorkSafeNB’s “Preliminary Accident Notification” documents, which are created when WorkSafeNB is notified of a workplace accident. Accident Reports are made public to New Brunswick’s employers and workers with the hope that the information will help prevent similar accidents from occurring.

Worker was climbing onto a roof from a ladder when the ladder kicked out and he fell approximately 12 feet.

While working on the dust eliminator, the worker reached into the unguarded area to pick up a rock and his coat sleeve caught between the rollers, pulling his arm in up to the elbow. The machine was immediately stopped and the fire department pried the rollers apart to remove his arm.

As worker was driving away after servicing a water pump his vehicle's ROPS caught the compressed air line, causing the projection of the compressed air hose and lubricator, which struck the worker in the face.

Worker was raising a buoy attached to underwater equipment with a hoist from a barge when the hoist cable broke and the buoy struck him. He died in the hospital 8 days later from severe head trauma.

Worker was having difficulty closing the dock leveler, so entered the truck trailer for better leverage. The worker fell from the trailer onto the ground when the truck driver, who was unaware the worker was in the trailer, started driving the truck.

Worker was using a mitre saw to cut fascia board and cut the tip of his finger off.

Worker was feeding sheets into a pressing machine; when the sheets did not return she looked in behind the machine and found that the rollers were not catching the sheets. As she pushed the material into the rollers under the guard, the sheets caught and pulled her hand into the rollers. A pry bar was used to free her hand.

Worker was pulling a 120-volt light switch cable from a conduit while standing on the second rung of an aluminum ladder when the cable shorted inside the conduit. The worker could not release the conduit/cable and kicked the ladder out from under himself. Still unable to release the conduit/cable, the worker kicked himself away from the wall, and fell backwards to the floor, finally releasing the conduit/cable.

Worker was unloading firewood from a dump truck; when the tailgate would not open he hit the gate with a sledgehammer. The gate released and the wood began to fall out, and, at the same time, the truck started backing up, pinning him under the tires. The parking brake was not on.

Worker was finished his shift and felt ill, so went to the emergency room where he was diagnosed with CO poisoning; he was treated and released. Upon hearing of the incident the following morning, the contractor shut down the site and sent 16 workers to the emergency room to be checked.

Worker had signalled to the forklift operator to pick up a load. He had turned his back away from the lift truck and the operator reversed, knocking him down and running over both legs.
