News Release WorkSafeNB investigates fatal accident in Long Reach – Richard Lively

Saint John, NB: The Workplace Health, Safety and Compensation Commission (WHSCC) is investigating an accident that claimed the life of a 41-year-old Home Hardware worker on Tuesday.

Richard Lively, of Grand Bay-Westfield, was travelling in a forklift from a work site when he lost control of the vehicle and landed in a ditch. Lively was thrown from the vehicle, and died at the scene when the forklift toppled over on top of him.

WHSCC officers were dispatched as soon as the accident was reported, and are still investigating to determine whether there were violations to the Occupational Health and Safety Act, and to make recommendations to prevent such a tragedy from recurring. At this time, no orders have been written or charges laid.
