Description: While delivering a large package (approximately one ton) to a residence, employee fell 1.8 metres from the back of the delivery truck and the pallet jack fell on the employee.

Description: Employee was walking on temporary bracing while installing trusses on a residential house when the bracing failed and the employee fell approximately 2.7 metres (9 feet) to the floor. The employee later died of head injuries at the hospital.
