Description: Worker lifted a small tractor through a hatch on the second floor of a poultry farm building using a chain hoist. He closed the hatch and began using the tractor to clean the floor. The structure around the hatch failed and the worker fell approximately 8 feet to the first floor, with the tractor landing on him.
