    Description: Employee was in a truck on a hoist at ground level. A second employee was in front of the truck and indicated to the other employee to start the truck. When the employee did so, the truck, which had a standard transmission, jumped forward, pinning the second employee’s arm.

    Description: While unhooking a trailer from a truck, the truck rolled back and crushed the worker beneath it.

    Description: Employee fell 2.4 metres (8 feet) to ground from scaffolding. The scaffolding had no legs, and was set up on a sheet of pressboard, which did not support the weight of the scaffolding and employee.
